Your Provider Relationship is the Key to Success
In 2021, RISE, Inc. acquired the contract with the Idaho Department of Health and Welfare to provide Post Permanency Services to families statewide.
RISE is a non-profit agency who is committed to the mission of providing opportunities for and with people. RISE knows that the relationship between family members and their provider is crucial in the overall success in services provided. That’s why staff work diligently to ensure families are matched with providers they can connect with who will help them achieve their goals.
If you’re interested in being linked up with a provider, please see our Find Provider page.
Frequently Asked Questions
What services are available for me and my family after adoption is complete?
There are a few services we offer to families. First, this website will serve as a resource tool for post-adoptive families, referring agencies, and community partners who are interested in supporting adoptive families. When families subscribe to our site or to our newsletter, they’ll get updates to this site and other useful resources/materials straight to their email inbox. Second, we offer assessment and referral services to all families who are interested in accessing additional support. This support could be for mental health referrals, advocacy/support navigating the IEP/504 process in school, or seeking a referral for a specialized provider. Lastly, we provide crisis services to families needing intensive support and stabilization.
I’m a collaborating provider in the community. How can I make a referral for these services or promote my training or support group on this webpage?
We welcome collaboration with community partners across the state. If you have a family you’d like to refer, or an event or support group you’d like us to list on our website, please contact our department by phone or email and a member of our team will be in touch.
How are services funded?
Services are funded through a mix of state and federal dollars. There are no charges for the services provided by RISE staff to post-permanency families. However, depending on the family’s medical insurance, fees may be applied to services which are referred by the RISE team for families to access to help to support their individualized needs (for example, you may be asked to pay a copay at the time of visit for the referred speech therapist RISE staff helped to coordinate with you and your family).
How can I request services?
Give us a cal at (208) 252-6117 or send us an email via the form on our contact page. Our staff will reach out promptly and coordinate with you to collect some information to determine how best our services can support you.
When a child turns 18, which benefits, if any, continue?
No benefits are available to children over the age of 18.
